\u2022 Inform student who will be their Lead Practice Supervisor\/s and Practice Assessor\/s
\u2022 Complete orientation checklist on e-PAD and any necessary local inductions on first day.
\u2022 Student submits orientation to either the Practice Supervisor or Practice Assessor to approve
\u2022 Arrange first initial interview date
\u2022 Set Ground rules for students<\/td>		<\/td><\/tr>

Week 1<\/strong><\/td><\/td><\/tr>
\u2022 Complete initial interview with Practice Assessor (this can be delegated to lead practice supervisor)
\u2022 Complete Learning Plan (student task then submit to Practice Supervisor or Practice Assessor to approve)
\u2022 Identify proficiencies which can be achieved on placement
\u2022 Discuss evidence (see below) required to sign off proficiencies and ePAD
\u2022 Identify learning opportunities
\u2022 Set date for Formative and Summative assessment
\u2022 Set dates for assessment of Episodes of Care and\/or Medicines management (give opportunity to complete formative prior to final summative assessment)<\/td>		<\/td><\/tr>
Week 6 – 7 Practice assessor<\/strong><\/td><\/td><\/tr>
Midpoint interview (formative) look at evidence gathered so far
\u2022 Student to complete self-assessment
\u2022 Practice Assessor discuss student\u2019s self-assessment and complete feedback
\u2022 Plan for completion of proficiencies, learning plan, review evidence
\u2022 Complete formative for Episodes of Care\/Medicines Management and student to complete reflection on ePAD
\u2022 Practice Assessor to complete feedback on ePAD
\u2022 ENSURE ALL BOXES ARE COMPLETED
Submit Midpoint interview and any formatives undertaken for Episodes of Care or Medicines Management
\u2022 Action plan to be completed if required (contact Academic Assessor)
\u2022 Mark any assessment of proficiencies as Achieved Yes or No and include the date the assessment took place
\u2022 Note if a student does not undertake an assessment leave it as blank. Marking it as Achieved No indicates the student failed the assessment rather than not undertaking it
REMINDER- SUPERVISORS CAN SIGN OFF PROFICIENCES<\/td>		<\/td><\/tr>
Week 8 – 13 Practice assessor<\/strong><\/td><\/td><\/tr>
Complete summative assessment of Episodes of care\/Medicines management<\/td><\/td><\/tr>
Week 13 – 14 (final week) Practice assessor<\/strong><\/td><\/td><\/tr>
Final interview (summative) as per Midpoint self-assess and feedback
\u2022 Student needs to have completed their reflections on Episodes of Care\/Medicines Management
\u2022 Review all evidence linking theory to practice
\u2022 Ensure all proficiencies that have been met are signed as YES\/NO
\u2022 Practice Assessor to complete feedback and complete OAR<\/td>		<\/td><\/tr>
If end of part (Year \u2013 after both placements) Practice Assessor to complete end of part section in OAR<\/td><\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n
